Moorfields Eye Hospital FT: Home Vision Monitor

Moorfields Eye Hospital introduced a service improvement initiative, the Home Vision Monitor, a smartphone-based app that empowers patients to test and monitor changes in their vision at home, during covid-19 lockdowns and beyond. This was the first large-scale deployment of home vision monitoring in real-life clinical practice. Patients are asked to test their own vision twice a week, with results sent instantly to their clinician at Moorfields. If both tests show any discrepancies, an alert is automatically triggered. This helps the clinician to intervene at an early stage of disease progression and provide patients with the chance of better outcomes.

The judges felt that this ambitious project is a fantastic display of how remote monitoring technology can empower patients and relieve pressure on the NHS.
