Vitality Partnership: Integrating information through Vitality Health Hub

The judges said: This project looks ahead to the future of healthcare. It has an ambitious approach to improve patient and staff benefits and is a great way to strengthen primary care capacity.

The winner of three HSJ Value Awards, the Vitality Partnership has redesigned the way in which patients are able to access their information and receive support.

In October 2013 the Vitality Health Hub was launched providing a fully integrated approach to digital healthcare across Birmingham and Sandwell. The service gives patients access to online, same day and extended hours services and also supports patients to be proactive in their healthcare.

Patients have access to personal care planning tools and long term management services. They are also able to view condition specific content and programmes of care all via the Hub website and app. Since the Hub’s launch these videos, which feature local doctors, have already had over 3,000 views. Electronic prescriptions are available as is online registration, practice information and opening times.

Integration is a key part of this project and through the hub patients are able to easily access community and speciality services. Currently 15 practices have been integrated in to the hub and since its launch it has had reduced DNAs by 70 per cent, takes almost 800 calls a day, and increased day capacity by 10 per cent.
