People experiencing severe and multiple disadvantage face extreme health inequalities, including long-term trauma, premature frailty, and early death. The complex challenges they present are difficult to address in isolation, creating significant demand, staffing and resource pressures that undermine the delivery of interdependent outcomes for partners across health and care systems.
Nottingham City Place-Based Partnership has established a comprehensive local partnership programme, involving people with lived experience, to jointly develop and deliver a sustainable, place-based model. This approach aligns efforts and responsibilities across the system to reduce health inequities while securing efficiencies for all sectors.
