Holistic Prescribing in Secondary Prevention: Optimising Outcomes Using an Innovative Cardio-renal-metabolic Service
The Gateshead Secondary Prevention Service represents the UK’s first cardio-renal-metabolic team dedicated to comprehensive risk factor management for patients following cardiovascular events. The service supports people who have experienced heart attacks, stable angina, stroke, transient ischaemic attacks, or peripheral vascular disease.
The service operates through a multidisciplinary model that is consultant-led, pharmacist-delivered, and patient-centred. It focuses on helping patients achieve recommended targets for cholesterol, diabetes, and blood pressure control using evidence-based medical treatments. The team provides supportive counselling to improve medication adherence during the crucial initial months after a cardiovascular event.
This collaborative and streamlined approach is designed to deliver better clinical outcomes, while providing value for money within the local health system.
