From data to action - Using population health management to improve population health and reduce health inequalities in hypertension management.

This project demonstrates how digital transformation can drive evidence-based intervention design by providing real-time insights into population health and health inequalities. Dorset’s system links live data from primary care, demographic sources, and deprivation indices to deliver actionable intelligence that helps identify variation, target areas of need, and reduce unwarranted clinical outcomes.

Interactive dashboards and data segmentation tools enable local teams to design personalised care pathways that focus on equity, particularly in hypertension management. Digital solutions, including remote monitoring capabilities, support scalable and proactive care delivery across the system.

This approach gives primary care teams the intelligence they need to lead transformation efforts. It embeds prevention and personalisation at a system-wide level while tackling health inequalities and improving outcomes for the entire population.
