Neighbourhood Health and Care – Transforming Care for Older People Through Place-Based Partnership

Surrey Downs Health and Care Partnership has replaced fragmented, hospital-based services for older people with an integrated, neighbourhood-led model.

Seven Integrated Neighbourhood Teams, aligned with Primary Care Networks, deliver proactive care with devolved budgets and shared leadership. The HomeFirst service provides hospital-level care at home, combining virtual wards, urgent community response, discharge-to-assess pathways and frailty beds into a single offering.

This transformation has empowered frontline teams, improved patient satisfaction and reduced hospital admissions by over 30 per cent, providing a scalable blueprint for delivering neighbourhood care closer to home.
