Pathway Improvement Project

A cross-disciplinary end-to-end pathway redesign for heart failure in north west London was underpinned by digital systems integration and innovative management practice. More than 100 interventions were combined to deliver substantial improvements including: data systems integration between primary, community and secondary care; increased community HF diagnosis, reduced unplanned hospital costs, greater convenience for patients and the first remote monitoring app for HF medicine optimisation in the UK. The culture of collaboration and innovation led to positive impacts for the entire spectrum of stakeholders and is applicable to other disease pathways. The judges said this was a very passionate presentation with excellent involvement. The winners displayed an impressive approach to design and the scale and transferability was evident.
