This community-based NHS 111 option 3 Palliative Care Hub Service is staffed by clinical nurse specialists providing improved access to palliative care advice, support, treatment, and signposting. It’s available to patients, carers, GP or other health professionals by calling 111 and selecting option 3. The CNS will discuss status and issues being experienced and can provide direct support or liaise with other services such as out-of-hours primary care, community nursing teams, social care, voluntary organisations, and acute consultants if required. The service is available 5pm-9.30am and 24 hours at the weekend and bank holidays.
