WINNER: Northamptonshire Health Foundation Trust and North and West Northamptonshire Council: Remote Monitoring Northamptonshire

Integrated Care Northamptonshire has successfully implemented an innovative remote monitoring programme aimed at helping older and frail people.

This has been delivered in a partnership by health and social care to target people in care homes and people in the community as part of the Integrated Care Northamptonshire Age Well programme.

The aim of the project is to use technology in the form of virtual health to monitor people in Northamptonshire in their place of residence and provide clinical support through a virtual hub at a time when they most need it, helping to reduce situations escalating.
