WINNER: NHS England - East of England, East of England Ambulance Trust, and East of England UCR Providers: Access to the Stack - Cleric Digital Portal Enabler for Transferring Patients to Urgent Community Response Teams

The implementation of the East of England Ambulance CLERIC Digital Portal has enabled Urgent Community Response (UCR) service provider teams to directly review and accept appropriate referrals from the ambulance service. UCR providers and ambulance service control room teams are working collaboratively with open channels of communication to ensure patients are responded to quickly by the right service at the right time. This project, successfully completed within a challenging short timeframe, has shown how the power of collaboration and willingness to embrace change has delivered improvement at scale for patient outcomes across the region.
