An Alliance Model of Dementia Care From Diagnosis to End of Life

The integrated dementia service holds all patients with dementia from diagnosis to end of life, ensuring ongoing and timely support by their named practitioner and supported by Coordinate My Care, a shared digital care plan to support crisis management. Patients diagnosed with mild Cognitive Impairment are also recalled for reassessment and timely intervention. This is an alliance-led collaborative between secondary care, primary care, the voluntary sector, urgent care, commissioners, local authority, patients and Coordinate my Care Partners to address the prevailing concerns about the lack of joined-up services and shared care planning. This partnership has a real sense of place, community and purpose, said the judges. Their approach has effectively harnessed the potential of several aspects that are rarely all mastered at the same time. A relentless focus on a clear goal, a real passion to work with people and communities.
