University Hospitals Birmingham has transformed its safety culture through Listen, Learn, Share (LLS) – a trust‑wide, compassionate approach to learning from patient safety events. Developed in response to significant cultural challenges, LLS promotes psychological safety, supportive learning processes, and open conversations. More than two hundred Learning Ambassadors now champion this work across all sites, supported by standardised tools, visible communications, and a central learning hub. Outcomes include improved staff survey results across every safety indicator, a more supported and resilient workforce. LLS is now an embedded, sustainable model that is reshaping culture and spreading across all sites and beyond the trust.
