PROACT – Community Led Parkinson’s Care

The PaRkinson’s Outreach Advanced Community Therapy Team (PROACT) is a community-based service, developed in collaboration with Parkinson’s UK, transforming access to specialist Parkinson’s therapy. It addresses preventable harm caused by delayed access to specialist Parkinson’s therapy, including falls, functional decline and avoidable hospital admissions. Through timely home-based interventions, the team has reduced waiting times by 90 per cent, reduced the cost of healthcare, improved quality of life and reduced the risk of complications, such as falls and fractures. PROACT enables proactive risk identification and intervention, demonstrating how integrated neighbourhood care can reduce adverse events and deliver safer, more anticipatory, responsive Parkinson’s care.
