From Crisis to Safety – A Scalable Inclusion Health Model for Refugees and Asylum Seekers

This service redesigned how healthcare is delivered to refugees and asylum seekers in temporary accommodation by moving care from emergency settings into coordinated, GP-led primary care delivered on-site. The trauma-informed multidisciplinary model reduces patient safety risks caused by delayed access, fragmented care and poor continuity. Over 12 months, 6,695 patients were supported through 11,134 clinical contacts, with 95 per cent managed without urgent care. The model improves patient safety, supports hospital discharge and reduces pressure on urgent and acute services, demonstrating that earlier, coordinated care is both safer and more cost-effective.
