Recentring, Tailored Patient Education Prioritised by Patient Need – A Digitally Enabled Community-Led Intervention

The project enhances patient safety by delivering heart‑failure education with digital monitoring by a community provider organisation. The organisers introduced new non‑clinician roles that digitally track self‑reported symptoms, provide personalised education using a co‑produced syllabus, and connect patients to community and social‑care support when their health deteriorates. This strengthens health literacy, enables earlier detection of deterioration, and ensures rapid escalation to safe, out‑of‑hospital alternatives. By shifting routine support into the community, the model increases service capacity, allowing specialist heart‑failure nurses to focus on complex care, supporting national priorities to move care closer to home.
