Financially Vulnerable Pensioners
Cold homes and financial vulnerability significantly increase health risks among older people, driving higher demand across primary care, emergency pathways, and community services. Dorset Intelligence & Insight Service created a digital cross-sector risk model integrating clinical data, GP records, social care indicators and financial vulnerability mapping to identify approximately 44,000 pensioners at heightened risk. The project connected NHS services, local authorities, social care, housing, and voluntary partners through a shared digital dataset and coordinated digital workflows. This enabled rapid, proactive support, targeted outreach, and improved care navigation for older people, forming a replicable model for joined-up winter resilience.
