Delivering the Left Shift - From Virtual Wards into Proactive Care with LLR ICB
Doccla and Leicester, Leicestershire and Rutland ICB have co-designed a proactive, digitally enabled out-of-hospital care service for people with complex long-term conditions. Using population risk stratification, remote monitoring and a virtual clinical hub, the model supports earlier intervention, strengthens primary and community care, and reduces avoidable hospital use. Patients and carers are supported to manage at home with rapid access to clinical advice and escalation when needed, improving independence, choice and confidence while releasing capacity across GP, ambulance, emergency and inpatient services.
