Pathways App: A Co-produced App for Collaborative Care Journeys
The Pathways App launched as a QI project in April 2022 within a medium-secure hospital in East London. Originally designed as a digital tool to support the planning and monitoring of recovery goals throughout a patient’s admission, the app now also provides a clear, visual overview of their care pathway, helping to enhance understanding and encourage active involvement in treatment decisions. It has since evolved significantly, through ongoing co-production, into a platform that also offers valuable insights into barriers, delays, and service gaps, providing a comprehensive top-down overview of the service.
