Mersey and West Lancashire Teaching Hospitals Trust wanted to avoid hospital admissions for high-risk groups. This prevents hospital-acquired infections, falls, delirium, deconditioning and a reduction in mortality. The virtual ward solution was utilised as an extension of the existing community frailty and urgent community response offer, seamlessly blending staff across all “place” areas with senior clinical oversight. This ensures continuity of care and expertise, and gives all patients fair access to the service from the comfort of their own homes.
