Diabetes Pilot Project – An Integrated and Collaborative Neighbourhood Working Initiative
The diabetes project demonstrated the effectiveness of a human factors approach in enhancing safety, improving outcomes and supporting patients to manage their condition. By considering the needs of patients, families, local organisations and healthcare providers, this methodology fostered collaboration and systemic improvements with patient safety at its core.
This collaborative model reduced potential errors while strengthening trust and communication between clinicians and patients. It ensured that all interactions, whether in person or remote, were designed to make patients feel listened to, understood and supported in making lifestyle choices.
