The Role of Pharmacists in Supporting the Triple Aim Framework for Individuals with Learning Disabilities

As pharmacists, they offer specialised expertise through home-based structured medication reviews (SMRs). These reviews are particularly valuable for patients with learning disabilities, as they ensure medications are optimally prescribed, appropriate for the individual’s needs, and clearly understood – all factors that improve adherence and ultimately lead to better health outcomes.
