Holistic Prescribing in Secondary Prevention: Optimising Outcomes Using an Innovative Cardio-Renal-Metabolic Service

A multidisciplinary health professional team would provide tailored care for patients, aligned with the Primary Care Network (PCN) Contract Direct Enhanced Service (DES) and the locally defined cardiovascular plans. This is the UK’s first cardio-renal-metabolic team to offer comprehensive risk factor optimisation for people who have experienced cardiovascular events, ie myocardial infarction, stable angina, stroke, transient ischaemic attack, or peripheral vascular disease.

The multidisciplinary, consultant-led and pharmacist-delivered model puts patients at the centre of their care. It supports people in achieving recommended cholesterol, diabetes, and blood pressure targets using guideline-directed medical therapies. Supportive counselling helps increase medication concordance in the crucial months following a cardiovascular event.
