Redesigning Parkinson’s Care with Digital Innovation

Leeds Teaching Hospitals partnered with Kneu Health to redesign Parkinson’s care into a proactive, digitally enabled pathway. Using a smartphone app and a clinician dashboard, patients’ motor, cognitive, and medication data are continuously monitored, enabling early intervention and personalised care. More than 12 months, 207 patients completed 40,000 exercises, 2,329 cognitive assessments, and 36,869 medication acknowledgements. Patient empowerment increased by 11 per cent, knowledge by 19 per cent, and confidence in self-management by 5 per cent. Remote consultations freed capacity for complex cases, saving £2,345 per patient annually. This evidence-based, clinically led redesign demonstrates measurable improvements in patient outcomes, efficiency, and sustainability, with successful replication in other NHS trusts and conditions.
