The Health Squad
The Health Squad provides outreach assessment and health navigation support to groups experiencing health inequalities, including people who are homeless, leaving prison, migrants, Gypsy, Roma, and Traveller communities, veterans and others, as identified by the integrated care board.
The team works with commissioners, partner organisations, local authorities, community venues, and strategic groups to identify and tackle emerging health inequalities. It delivers frontline support in community settings to help people engage with and access healthcare services. This includes health assessments delivered to clinical standards in community settings, and a four-tier system that assesses individual needs and identifies appropriate care pathways for each person.
