Reducing Health Inequalities for Working-Aged People Living With Diabetes in Slough Through Point of Care (POC) HbA1c Testing. A Core20PLUS5 Project in NHS Frimley.
This joint initiative between NHS Frimley Health, Frimley Integrated Care System, and Lilly supports the Core20PLUS5 strategy to tackle health inequalities in Slough. The project has improved access to diabetes care for working-age adults by introducing point-of-care HbA1c testing, which removes barriers such as multiple GP appointments, time off work, and childcare difficulties. The service is delivered through one-stop clinics and home visits by the multigenerational household team. Results show it has improved access and engagement, leading to better clinical management. The project aligns with NHS and Frimley ICS priorities and offers a model that could be scaled up for managing long-term conditions in underserved communities.
