Transforming Access to Healthcare for Homeless and Vulnerable Patients in Sefton
Primary Care 24 and Light for Life have developed a nurse-led service at North Park Practice in South Sefton to improve care for people experiencing homelessness. Patients are identified through hostels and outreach, registered with a named GP, and receive a medication review within one week. Supported appointments and a trauma-informed approach have reduced missed appointments and rebuilt trust. The service has cut accident and emergency attendances, improved prescribing safety, chronic disease monitoring, and strengthened quality and outcomes framework compliance. The model is now expanding across commissioned accommodation.
